TheraPlatform offers built-in consent form templates, which are intended as general samples. These templates have not been reviewed by legal experts and must be customized to meet the needs of your practice and comply with state-specific regulations.
The account administrator or a biller with permissions granted by the administrator can set up consent forms under Settings. Once set up, administrators, therapists, and staff members can send these forms to clients.
Clients can securely sign consent forms through the client portal. All completed forms are automatically saved under the client’s chart.
How to edit built-in consent forms
Step 1: Go to Settings > Forms > Consent > Browse Library
Step 2: In the pop-up window, click the eye icon next to the consent form you’d like to preview.
Step 3: Carefully review the sample consent form. Once finished, click Close at the bottom of the form.
Step 4: If you want to use or edit the consent form, click Select next to the form title.
Step 5: The selected form will appear in the white area below, displaying its title and three icons:

Pencil icon to edit

Trash icon to delete

Square icon to duplicate
tep 6: Click the pencil icon to edit the form.
Step 7: A pop-up window will appear where you can make changes to the form.
Step 8: Make sure the toggle switch labeled Enabled is turned on (purple). This ensures the form is available for staff and therapists to send to clients.
Step 9: You can edit the title of the consent form in the Title field.
Step 10: To edit the main text of the form:

Click the pencil icon next to the Agreement section.

You can delete the sample text and add your own or modify it as needed.
Step 11: Once you're done editing, click Close, then Save Changes.
Step 12: To add additional fields or edit our fields, use the tools in the form builder.

How to Add Footers to Your Consent Forms
Step 1: Go to Settings > Notes > Settings
Step 2: Follow the on-screen instructions to enter the desired footer text.
Step 3: Click Save Changes.
