TheraPlatform offers a library of consents and form builder that allow you to build any form from scratch to meet your needs.
Once you set up consent forms under Settings< Forms< Consent, you will be able send them over to your clients via a secure client portal.
There are two ways of sending consent forms to your clients:
- You can send consent forms to your client when adding a new client to your caseload or
- You can send consent forms at a later time when the client is already on your caseload.
New Clients (when adding a new client)
To send consent forms when adding a new client:
Step 1: Click on Clients< Add.
Step 2: A pop-up box will open, where you can enter the client's information.
Step 3: To send the consent forms, select all the consent forms you wish to send in the field Consent Forms. All enabled forms will appear in the box. If you do not see the form you added, navigate to Settings > Forms to enable the form you would like to see there.
Step 4: Click on Save changes.
Note: Once you click Save changes, the client will receive an email notification from TheraPlatform informing the client that you shared consent form/s. The client will be asked to log in to the client portal to review them and sign them. Once client completes them, the forms will be automatically saved for you under client’s chart under Documents.
Existing clients (from the chart)
Step 1: Click on the Clients.
Step 2: Click on the icon of the eye or start typing the name of the client in the top Client search bar. Select the client and this will open the chart.
Step 3: Click on Documents< Consent Forms.
Step 4: Click on Send Consent Forms button and select all the forms you wish to send.
Step 5: Click on the Share Now button to send them.
Note: After your consent forms are sent, your client will be able to complete these forms after he/she signs in your client portal. When forms are completed, the forms will automatically save under the chart. You can view the forms in the chart under Documents< Consent Forms.
Important: TheraPlatform has built-in consent forms that you can use. Before you send consent forms, you MUST review them and modify them to fit your needs. Click here to learn how to edit form templates.
