How to Add Footer to Note Templates, Intake and Consent Forms

You can insert specific patient and referring physician details directly into the footer of your notes and forms. This feature automatically pulls key information from the patient’s profile or referral section, helping you save time and reduce repetitive data entry.

New Smart Fields Available for Footers

You can add the following bracketed smart fields to the footer of your notes or forms:

• Patient's Address: [PatientAddress]

• Patient's Phone Number: [PatientPhone]

• Referring Physician’s Name: [ReferringPhysician]

• Client's Name: [clientname]

• Client's Date of Birth: [clientdob]

• Patient's account number: [account_number]

When these fields are added, TheraPlatform will automatically populate them with the corresponding data stored in the patient’s profile or referral details.

How to Use These Fields

Step 1: Log in to your Virtual Practice.
Step 2: Navigate to the Settings module.
Step 3: Click on the Notes tab.
Step 4: Select the Settings sub-tab.
Step 5: In the footer section, start typing a bracket “[” — a list of available dynamic fields will appear for you to choose from.
Step 6: Select the desired dynamic field(s), such as [PatientAddress], [PatientPhone], or [ReferringPhysician].
Step 7: You can also add custom text along with the dynamic fields if you’d like to include additional details or labels.
Step 8: Once you’re finished, save your changes to apply the new footer layout.
