Consent forms

Consent forms  (consent) that are offered and built into TheraPlatform can be found under Settings< Forms< Consent forms.

You can modify our consent forms and your can build your own consent form from scratch. 

Note: The signed by clients consent forms, will show electronically signed by (client's name) and IP number + the actual signature of the client + electronically singe by (clinician's name)

P.S. If you'd like consent form to display the actual signature of the clinician, make sure that the clinician has saved their signature under My Account<Details.
