If you are a patient who is having difficulties with a demographic form such as not able to submit it, or the continue or save later or move forward buttons don't work, it could be due to requested filed that was left blank/empty. Make sure that you enter information under requested fields. If you cannot provide information under requested fields, the demographic form will not be saved/submitted. In this case, contact your provider directly and let them know to modify the demographic form so it does not request the field.
