How to Convert Your Own PDF Intake or Consent Forms into Digital Forms

You can upload your existing PDF intake or consent forms to TheraPlatform and have them automatically converted into digital forms. This AI-powered feature eliminates the need for you to manually recreate forms using the form builder.

Pricing: This feature costs $5 per PDF page converted. Charges are based on the number of pages in the uploaded document.

How to convert  your PDF into a digital form

Step 1: Log in to your TheraPlatform account.

Step 2: Go to Settings → Forms.

Step 3: Select either Intake Forms or Consent Forms, depending on what you're converting.

Step 4: Click Upload to Convert. In the pop-up window, attach your PDF by either clicking Browse Files or dragging and dropping your PDF into the upload area.

Step 5: Wait for the conversion to complete. This may take a few seconds, depending on the number of pages being converted.

Step 6: Your converted form will appear and to review the form, click on the edit button next to the title of the form.

Step 7: To see a preview of the form, click on the Preview button.
