TheraPlatform allows administrators to customize the Contact Form that clients complete as part of their records. This form is used to collect important information about a client’s emergency contact. Administrators can edit which fields are required before a client can submit it.
Customizing the contact form helps ensure you collect only the information that’s relevant to your practice. For example, you may require a contact’s Mobile Phone Number but not their Street Address — in this case, you can mark the mobile number as a required field and leave the address field optional.
How to Customize and Edit the Contact Form
Step 1: Log in to your personal TheraPlatform URL.
Step 2: Click on Settings.
Step 3: Select Forms.
Step 4: Click on Contacts.
Step 5: Use the toggle switch next to each field name to mark it as required.
Step 6: Click Save Changes to apply your updates.
When a switch is turned blue, that field becomes required.
For example, if you want to make Date of Birth (DOB) a required field, click the switch next to it so it turns blue.
When clients receive the form through the Client Portal, they will not be able to submit it until all required fields are completed.
Sending the Contact Form to Clients
You can also send the contact form to clients who already exist in your Platform.
If a client already has information saved in their record (for example, an address), that data will automatically pre-populate into the corresponding fields on the contact form when they receive it.
Clients can review and confirm the pre-filled information or update it as needed before submitting.
