If you’ve entered multiple diagnostic codes under the Assessment and Treatment Plan, these codes will carry over to the CMS-1500 form. If multiple CPT codes are present, each will be associated with diagnostic pointers (e.g., A, B).
Important Note:If your claim is not in Draft status, you must reset it to Draft in order to edit diagnostic pointers. Once you've made your changes on the CMS-1500, click “Update” at the bottom of the form to apply them.
If you need to switch the pointers for services on CMS-1500 follow these steps:
To Switch or Edit Pointers on the CMS-1500:
From Billing (Draft Claim – claim created but not submitted yet):
-
Go to Billing > Claims (use filters to help locate the draft or specific claim).
-
Click the eye icon to open the claim.
-
In the CMS-1500 pop-up, hover over the CPT code and click the pencil icon.

In the pop-up editor, you’ll see the CPT code and the associated diagnostic codes.
Remove any diagnostic codes that do not apply to that CPT code and click Save Changes. -
