Customize and Edit Demographic Form

TheraPlatfrom allows you to request from /send the demographic form to your clients. The demographic form can be edited by the administrator of your account and the biller of your account.

Customization of the demographic form allows you to check all the fields that you require from your clients. For example, you might need your clients to enter their address, but you don't need their office phone number. In this example, you can mark addresses as required. 

Customize and edit demographic information

Step 1: Log into your personal URL.

Step 2: Click on Settings.

Step 3: Click on Forms.

Step 4: Click on Demographics.

Step 5: You can mark any fields as required by turning the switch to blue next to the name of the field. For example, if you want DOB to be required, click on the switch next to it to mark it in blue. In this example, if you send demographic form over to your clients via client portal the client will not be able to save the from till required DOB is entered.

Step 6: Click on Save changes.
