TheraPlatform offers built-in intake form templates, which you can modify to meet your needs, or you can build any intake template from scratch.
Our intake template builder offers a toolbox with different options for the fields, including the ability to add a table.
Here are directions on how to add a table to our built-in intake forms:
Step 1: Click on Settings > Forms > Intake
Step 2: If you previously selected our built-in intake form from our library and now want to add a table to it, find the template in the white area and click the edit button.
Step 3: If you haven't enabled our pre-built template, click on Browse Library and select the template by clicking on Select.
Step 4: The selected template will drop to the white area; click on edit next to it.
Step 5: At the pop up box, click on the icon of the table to add a table to this template.
Step 6: The table tool will drop at the end of the template. You can move the location of this table by dragging and dropping it.
Step 7: Enter the label's title (This text will show above the table.)
Step 8: Next, click on the edit icon next to the title of the table label.
Step 9: Enter the number of columns in the columns field. If you wish to add row headers to the table, toggle the checkbox for "Row headers" and specify the # of row headers you would like
Step 10: Enter text (label for each column and row).
Step 11: Click on Save Changes.
Step 12: This will close the column pop-up box. You can preview the template by clicking the preview button.
Step 13: Click on Save Changes to save the entire template.
NOTE: The edited version of the table looks different than the printout version.
How to add a table when building your intake form from scratch:
Follow all the steps above except for Step 3: Click on Add From, and at the pop-up box, enter the title, mark all options applicable to your needs, and add all the fields of your preference (e.g. text, paragraph, multiple choices, table, etc.) using the toolbox.
