I was not referred by the physician

If you're a patient filling out the demographic form and you're asked to provide the name of the referring physician, but you were not referred by the physician, you can leave this filed blank.

However if the field referring physician is in red, this means that the name of the physician is required by your provider. If you leave this blank you will not be able to save the form. In this case, since you were not referred by the physician, reach out to your provider so the provider can modify the demographic form for you not to require the name of the physician.

The provider would need to go to the Settings< Forms< Demographic form and uncheck require referring physician.
